Department Paid Ordering Instructions:
If at any time you have any questions about phones, plans, ordering information, forms, etc... - Click Here (.XLS) to access the Sprint/EMBARQ™ Wireless Phone Order Form. This form is for Staff and Faculty use only and is available to provide an easy way to capture the information needed for your order (and can be saved to use as a template for future orders). Please complete the form as much as possible.
- Attach the Sprint/EMBARQ™ Wireless Phone Order Form to a UNLV Requisition (RX) and send to UNLV’s Telecom department. The Telecom department will review and forward to Purchasing to have a PO generated for Sprint.
Purchasing will then send the PO and your Order Form via email to UNLVorders@embarq.com or via fax to (407) 814-6702, so EMBARQ™ can process the order for shipping.
